Bad Ending
It is said that there are multiple bad endings - and literal endings, at that, meaning it goes to the credit screen and doesn't just say "GAME OVER." There may be more conditions, and if so, please contact me here.
Here are the conditions for the bad ending:
Condition 1 - If you constantly use the "fix" option instead of retreiving Shinen and miss enough optional Silents, the Hazard rate (displayed at the top bar when you are in the map view; a percentage) goes up and Fort will come into clear the city. (Extra information: Nola says in the beginning of the game that when the Hazard Level reaches 80%, that is when Fort steps in. Not confirmed).
Condition 2 - If you fail to stop Yui at the Police Station when Lieutenant is being taken away, Atsuki will be called back to HQ (Extra information: this ending is alluded to when Rui speaks of her fortunetelling, where she claimed to have seen her sister shoot many people. Confirmed).

Normal Ending
This is the "normal ending," although I find it quite horrific and should be labeled as a "bad ending." There's really only one condition in which you can get the normal ending.
Ending: Chapter 19, right after you defeat Edward Steiner's Silent.
Conditions: If you've missed Yumiko's and Honoka's Silents (optional), but haven't made any other mistakes (Extra information: it is said that Honoka's Silent is really the only one you need to look out for. To do this, continue going to the Nurse's Office whenever you can and get Shinen. In Chapter 16, for the next part to work, you MUST talk to Honoka in the Nurse's Office first! Then, in Chapter 18 you have to get Honoka's Silent when she's at Rainbow Hill, otherwise it will not work. Not confirmed).
Ending: Chapter 19, right after you defeat Edward Steiner's Silent.

The good ending is one you get regardless of what choices you've made when you play through the game the first time (it only makes sense because the first time you play it, you'll get all the information, characters and storyline to choose decisions and get different endings).
Ending: Chapter 21, a little while after defeating Graham Miller's Silent.
Conditions: You have to be very careful in your decisions to get this ending after the first run-through. You will only get this ending if you capture Yumiko or Honoka's Silent (Extra information: if you miss both of the Silent, you are permanently locked into the "Normal Ending" until that run-through of the game is complete. Confirmed).
As stated in normal ending, Chapters 16 and 18 are pivotal chapters in the game, and retrieving both of Honoka's Silent is mandatory for getting the good ending.
